The Need for a Specialized Children’s Cancer Hospi


“Somewhere, children dance to the joyous music of life, and elsewhere they only cling to existence….they are all ours” Unknown

Egypt’s population between 0-19 years of age in 2005 was projected to be 43% of the total population of 74 million. The US government SEER report, “Cancer Incidence and Survival in Children and Adolescents, 1975-1995”, stated that there is 150 children per million under 20 years diagnosed yearly with cancer. According to the WHO in their Global Action Against Cancer 2005 document, there are over 160,000 children globally diagnosed with cancer yearly, with 90,000 children dying yearly; 80% of these children live in developing countries.
According to the SEER report, the probability of developing cancer before the age of 20 years is 1 in 300. Therefore, each year it is estimated that there are approximately 8400 children in Egypt estimated to develop cancer. Forty percent of these children will die due to lack of adequate treatment and facilities, as well as advanced technology. Today in the West, as a result of intensive treatments, state of the art facilities, and a specialized pediatric oncology healthcare team; 80-85%% of children survive cancer. Egypt, the Middle East and Africa need a centre of excellence dedicated only for children with cancer to achieve these rates.



More Than Just a Children's
The AFNCI committed itself not only to construct the first children’s cancer hospital in this region but to develop a comprehensive healthcare system that is sustainable where management and treatment utilize the most scientific approaches practiced today. This is the only way survival rates will ever improve for children with cancer in Egypt.

The AFNCI and NCI collaborated with both the private and public sector on a national and international level to assist in realizing their goal of a children’s cancer hospital that would meet international standards for children’s cancer programs as well as children’s hospital design.

Design and Planning
Planning for the Children's Cancer Hospital began in 1999. Dr. Ahmed Fathy Sorour initiated the acquisition of land from the Cairo governorate with former Governor Abdel Rahim Shehatta and Dr. Sorour’s NGO, The Egyptian Organization of Social and Cultural Development. The land which was the former site of the slaughterhouse, situated in historic Sayeda Zeinab adjacent to the ancient aquaducts, had been slated for a park. However, both Dr. Sorour and Governor Shehatta agreed it should be used for the hospital but with the stipulation of keeping half of the 20,000 square meter piece of land as a garden. The CCHE 57357 planning team happily agreed to this arrangement which would ensure green space for the long term.

In June, 1999 HE Suzanne Mubarak performed the official ground breaking ceremony that launched the beginning of the innovative project.

The hospital was built according to the most up to date Western standards developed by an international team of experts from the United States, Sweden, Germany, Lebanon and Egypt who are highly specialized in hospital construction and known for their quality of work. An American health planning consortium known as SLAM was chosen to undertake the pre design phase. The SLAM team consulted with all NCI departments, AFNCI Board of Directors and staff, NCI administration, community leaders, Cairo and Egyptian government representatives and patients and their families. This extensive review resulted in:

demographic, market and demand analysis

operational and governance guidelines

functional programming and architectural design concepts

financial and budgetary guidelines and estimates for the project

staffing and equipment specifications

patient and family focused design
Design Phase
The design phase began with eminent healthcare facility architect, Jonathan Bailey. Mr. Bailey’s concept was to create a design that was futuristic symbolizing a new era in healthcare that could compare architecturally and operationally with international cancer facilities; but also respecting Egypt’s rich culture and architecture

Mr. Bailey and his team created a design that was elegantly childlike to inspire hope to children and their families who are suffering the devastation of cancer. The design also followed the important methodology of “Form following Function” to be user-friendly for patients and staff, and responsive to patient care, education and research needs in the coming decades. Mr. Bailey's design uses the latest concepts in acute pediatric oncology care, to produce a facility that is child-friendly, cost-effective to operate and maintain, and technically state-of-the-art based on western standards.

Exterior
The hand carved sandstone exterior and its shape of the famous Egyptian fellucca sailboat symbolize the deep ties with the past and pays homage to the rich Egyptian history as leaders in medicine and engineering.
The other concern was to have an exterior that would be able to withstand the extreme environment of dust and sand in Egypt.
The glass is easier to clean and maintain with the double pane system providing a high level of energy efficient insulation against the extreme weather conditions while the green colour acts as a UV filter allowing a great amount of natural light inside.
Research has shown natural light is important for the emotional well being of people and particularly important in healing.

Vision of the ball
The ball signifies the world and that the Children’s Cancer Hospital Egypt 57357 is part of the world in receiving the best knowledge and technology from the world community and sharing their knowledge so that all children with cancer both in and out of Egypt as well as healthcare providers will benefit.
The wide use of glass signifies the CCHE vision that our limit is the sky in treating our children to give them hope for a better future as well as indicating a feeling of transparency in all aspects and being open to all of the best care that the world has to offer.

Interior Design
The interior theme, widely utilizing natural light throughout and the selective use of colour, is intended to make the hospital an appealing place for children who will feel a sense of hope, security and a home-like atmosphere. Materials such as terrazzo and Corian were used to create a building that would be easily maintained, durable and infection control free. In compliance with the benchmark standards for pediatric oncology care, the patient rooms are single or semi-private to reduce the risk of infection for our immuno-compromised patients. Full services are being offered to achieve the standards of a pediatric oncology service which state that there should be a complete diagnostic centre, surgical and multi-disciplinary specialties to treat the broad spectrum of concerns that children with cancer have.

Features include:

180 beds with expansion capabilities to 350.

A design that facilitates family centred care

Specialized clinical pharmacy.

Intensive Care and Bone Marrow Transplant units.

A comprehensive surgery department able to conduct complicated tumour surgery such as neuro, micro, and ophthalmic surgery.

A large outpatient facility able to accommodate 300 patients per day.

Multi speciality clinics such as pain, cardiology, dental and ophthalmology.

The first specialized department of psychology, social work and psychiatry in Egypt for pediatric oncology patients.

The first child life and school program in Egypt for hospitalized children.

A comprehensive physiotherapy department for oncology patients in Egypt.

State of the art diagnostics and laboratory support:

§ CT, MRI, PET/CT scans, nuclear medicine, blood bank, cytogenetics laboratory, virology, cord blood storage and stem cell collection and storage, tissue bank and bone marrow bank

A fully equipped radiotherapy department.

Designed to meet the most rigid requirements for immuno-compromised patients and infection control

Designed to meet the highest standards for environmental issues such as waste and water management and energy consumption.

The design of the hospital meets the current recommendations for children’s hospital designs that were recently set by the National Association of Children’s Hospitals.
All private rooms as large as possible between 275 and 375 sq. feet including full bath, sleeping area, large screen tv with internet access, desk, refrigerator, windows from corridor and good view to outside

Cluster rooms in groups of 8-10-12 with control area for staff and parents and complete visibility into rooms

Include decentralized nursing staff charting areas with observation and privacy options

Provide space and benefits that recognize the importance of the staff which enhance recruitment and retention such as lounges and locker space.

Provide all necessary features that will encourage parents to stay and participate in care.

Design a facility that will appeal to all generations both staff and visitors

Recognize the importance of art and create a permanent collection that continues to grow and acts as a therapeutic agent.

Develop areas that have themes and have good use of colour

Recognize the importance of the lobby as being the second most important impression after the exterior

The exterior should not look like a hospital or institution
Construction and Development Phases
Construction was divided into packages and no package was tendered until the funding was secured. All companies were very committed to providing the best offer for the best price because they shared the vision of the hospital realizing the potential for helping the children but also in sharing in the new era of change. During each phase, the value engineering process was utilized to ensure that we had best value for our money. This process saved millions of dollars for the hospital. The philosophy of acquiring the best expertise to proceed through the phases of the project actually contributed to capacity building and knowledge transfer to local Egyptian consultants in building a world class healthcare facility who have now acquired the expertise for future healthcare projects in the region.

Equipment Plan
The equipment plan was a very complicated job because it was outfitting an entire hospital from scratch. The primary criteria was that the list had to include all the equipment that is considered standard requirements for a pediatric oncology hospital. For example all the necessary diagnostic equipment that is considered benchmark for diagnosing children with cancer had to be included such as sophisticated radiology equipment like the PET/CT, MRI,CT scans and all the different departments in the laboratory such as cord blood lab, cytogenetics, immunophenotyping and molecular biology. State of the art operating room equipment that could assist in performing complicated procedures such as neurosurgery, microsurgery and eye surgery was also necessary as was durable beds with multiple positioning and the highest quality mattresses for our immune compromised children who suffer major skin breakdown. In total there were 8000 pieces of equipment that needed to be purchased through the Equipment Tender. As mentioned previously both Siemens and ECRI have been involved in the implementation of the equipment plan with the UHS project managment and CCHE biomedical engineering. This is an example of true collaboration for the ultimate goal of establishing a world class hospital system.
Dream Team
Our state of the art hospital would never have become reality if it wasn't for the expertise of the many companies and consultants who worked on the project. It is truly an international endeavour and work of love. They form the core of the Hospital 57357 Dream Team. These companies and consultants saw the potential of what Hospital 57357 could do for Egypt and the world in healthcare and they did their best to help us achieve the dream. They understood that it was a unique project and so offered their best price without sacrificing quality.

Following are brief profiles of all the companies who have been involved with Hospital 57357 from the beginning outlining scope of work .
Dateline
1995: a volunteer group began to gather information about building a hospital

1996: meeting with Mofid shehab, president of cairo university

1998:
• Association of Friends of the National Cancer Institute registered and helping NCi achieve its goals. Serious discussion about building a hospital began
• First seed money,8 million LE was donated by several prominent Egyptian businessmen

1999: land was provided by the Cairo Governorate through the efforts of HE Fathi Sorour, Speaker of Parliament and Cairo Govenor Abdel Rahim Shehetta

July 1999: Egypt’s First Lady Mrs. Suzanne Mubarak officiated at the ceremonial ground breaking

August 1999: SLAM began pre-design phase

April 2000: Design phase began with Jonathan Bailey Associates, architectural firm and Skanska as project manager.

November 2000: work began on the property with the building of the construction site fence followed by a fence painting party attended by several hundred school children

December 2000: site excavation

April 2001: a tender was released for the foundation.

July 2001: construction began on the foundation
Ground water made building the foundation a lengthy and expensive process

November 2001: the tender for superstructure was prepared

March 2002: Superstructure begun
Construction is divided into packages and no package is started until the necessary funds are raised

March 2003: international RFP for interior released

June 2003 : super structure completed

January 2004: The establishment of the Children’s Cancer Hospital Foundation (CCHF) headed by Mrs. Suzanne Mubarak.

March 2004: electrical-mechanical and interiors phase begun

July 2005: Equipment tender released

August 2005: Executive Board of Hospital appointed to begin start-up of hospital.

February 2006: Equipment tender awarded to Siemens

August 2006: Jonathan Bailey Associates re commissioned to execute the interior design.

August 2006: Issuing of tender for Hospital management start-up.

December 2006: Equipment tender scope of work finalized and orders underway

February 2007: Awarding of Hospital Management Tender to Comprehensive Care International

March 2007: First pieces of equipment have arrived with complete amount arriving by end of 2007.

March, 2007: Non medical furniture arriving.

April, 2007:
-CCI begins work of management phase.
­ Hiring of staff begun.

April 15, 2007: Mrs. Suzanne Mubarak makes her first official visit with the First Lady of Armenia, Mrs Koucharian.

July 7, 2007: soft opening of hospital, first patients accepted.

August, 2007: 2.5 million LE training grant from Social Development Fund received and training programs being developed.
Training begun for all levels of staff.

December, 2007: Information Technology Contracts signed with installation and integration to take place over 18 months.

January, 2008: Mrs. Suzanne Mubarak visits hospital with the First Lady of Turkey, Mrs.
Cerner and Oracle begin the process of implementing the HIS and ERP system.

February, 2008: Mrs. Suzanne Mubarak visits hospital with Her Royal Majesty, Queen Sofia of Spain.
The first CCHE 57357 neuro-oncology visit and symposium: Visiting Professors from Harvard: Dr. Mark Kieran, Director of Pediatric Neuro-oncology, Dana Farber Cancer Institute, Children’s Hospital Boston, Dr. Peter Black, Founding Chair, Neurosurgery, Children’s Hospital Boston, Dana Farber Cancer Institute instructed staff about pediatric brain tumours and treatment strategies.

April, 2008: Dr. Jan de Kraker, Pediatric hematologist/oncologist , prominent specialist in children’s kidney tumours and founding member of the International Society for Pediatric Oncology[SIOP] from Amsterdam to instruct medical care team on kidney tumours current and future strategies and to consult on cases.
CCHE 57357 awarded the United Arab Emirates Health Foundation Prize sponsored through the World Health Organization for its significant contribution to the healthcare field.

May, 2008: Dr. Liliana Goumnerova, Pediatric Neurosurgeon, Harvard University, Children’s Hospital Boston to teach new techniques for brain tumour surgery to the team of CCHE 57357 neurosurgery.
June, 2008: Dr. Andrew Koh, Pediatric Oncologist/Pediatric Infectious Disease, Harvard University, Children’s Hospital Boston to teach Infectious Diseases in Pediatric Oncology Management and Infection Control Management.

July 7, 2008: CCHE 57357 is one year old!
HE Minister Mahmoud Mohie Din visits the hospital and promises to help the hospital.

August, 2008: Grand Mufti Aly Gomaa visits the hospital.
The National Football Team of Egypt visit the children and hospital with the Egyptian Football League Executives. They committed to giving 3 LE from each ticket from every National team game to Hospital 57357.
